For temporary vehicle benefits, all drivers must possess a current permanent drivers license issued by any state in the USA. NO EXCEPTIONS. Please email a scan or photo of all drivers licenses to: autoministry@gmail.com.

I hereby state that I am a current or prospective eligible driver of Auto Ministry Inc. (the “Company”) and authorize the Company or its agents to obtain my driver record from the issuing state, to be used by the Company or its agent to determine whether or not I should be allowed to operate a motor vehicle upon the public highways of the United States and/or for the purpose of underwriting insurance in connection with such operations. I agree to obey all state laws, and to not text while driving. I further understand and acknowledge that no information contained in the driver record shall be divulged, sold, assigned or otherwise transferred to a third person or party. I authorize the Company or its agent to use electronic means, such as email, to communicate the contents of this release or the driver record report to or from the Company or its agent. A photographic, faxed or electronic copy of this authorization shall be deemed as valid as the original.
